Efficacy of quantitative analysis for differentiating pancreatic solid lesions using contrast-enhanced endoscopic ultrasonography

Aim: To evaluate the efficacy of quantitative analysis for differentiating pancreatic ductal adenocarcinoma (PDAC) and inflammatory pancreatic masses (IPM) using time intensity curve (TIC) analysis based on contrast-enhanced endoscopic ultrasonography (CE-EUS). Methods: We reviewed 89 patients who had undergone CE-EUS for pancreatic solid lesions at our department between August 2012 and January 2016. CE-EUS images were recorded for 2 minutes after injection of the contrast agent. The diagnostic abilities of the enhanced patterns and TIC analysis were assessed. Results: The enhanced patterns of PDAC were mainly hypovascular and heterogeneous (66/77), while IPM were mainly isovascular and homogeneous (6/12). In PDAC, sensitivity was 77.9%, specificity 83.3%, and accuracy was 78.7%. In TIC analysis, the intensity reduction rate was significantly different at 10 and 30 seconds after peak intensity. After creating a cutoff value (49%) based on the receiver operating characteristic curve for the intensity reduction rate after 30 seconds, diagnosing PDAC with TIC analysis had a sensitivity of 67.5%, specificity of 100%, and accuracy of 71.9%. Combining enhanced pattern analysis with TIC analysis had a sensitivity of 90.9%, pecificity of 83.3%, and accuracy of 89.9%. Conclusion: Combining TIC analysis with CE-EUS improved diagnostic accuracy when differentiating between PDAC and IPM.

地域住民の健康づくり推進のためのライフスタイル質問票の開発と応用

地域住民が疾病・生活習慣リスクを知り、自己の生活を改善して健康づくりを推進するためのツールを作成するために、ライフスタイル質問票を開発した。質問票は8つの大項目（1.食事、2.食行動、3.身体活動、4.睡眠、5.社会交流、6.ストレス対処力、7.心理状態、8.健康状態・既往歴・経済状況・教育等）から構成され、質問項目の総数は82であった。質問はタブレット型PCで表示し、選択肢をタップ入力することによって回答できるようにした。回答に要する時間は10-25分であった。質問票の中から、「食行動」、「身体活動」、「睡眠」、「心理状態（こころの健康）」、「社会交流」の5項目に関係したものを選び、それぞれの項目について個人の状況を5段階でレーダーチャートによって示し、「健幸バランス度」として可視化した。レーダーチャートは質問票に回答した直後にタブレット型PCに表示させて即座に本人にフィードバックすると共に、後日専用用紙に印刷して本人に返却した。ライフスタイル質問票は健康診断の結果と合わせて分析することにより、生活習慣病の罹患リスクや健康行動の効果を個別に通知し、生活習慣の振り返りを通して健康のための行動変容を促すことができる。

アルコール依存症から回復しやすい社会になるための考察

アルコール依存症は慢性・進行性の精神疾患であり，治療につながりにくく，回復が困難であることが指摘されている。また，アルコール依存症は，「孤立の病」ともいわれ，アルコール依存症に対する社会的偏見・誤解もある。アルコール依存症やアルコール関連問題の正しい知識や理解が普及し，誰もが孤立することなくアルコール依存症から回復しやすい社会が求められている。

スナヤツメ成魚の好中球の形態学的および細胞化学的特徴

Neutrophil granules (NG) of adult (mature) far eastern brook lamprey Lethenteron reissneri [L. sp. S (souther form)] were classified into two types of stratified [two-layer; inner layer (L0) and outer layer (L1)] granules (type 1, NG1a; type 2, NG2), like as larva (ammocoetes) of this species. The L0 of NG1a and L0 of NG2 in adult neutrophils were chromophobic, and L1 of NG1a and L0 of NG2 showed chromatophilic. The stainability of those chromatophilic layers with May-Grünwald (MG), Giemsa, or MG-Giemsa were varied [chromophobic, eosinophilic or basophilic (orthomethylenophilic or metaazurophilic)] with a slight difference to larva. As seen in larval neutrophils, these layers were positive for alkaline phosphatase,α-naphtyl acetate esterase and Sudan black B, and some enzymes (acid phosphatase, ꞵ-glucuronidase, and naphthol AS-D chloroacetate esterase) were detected in the L0 of NG2. Alpha-naphtyl acetate esterase was positive in L0 of NG2 of adult neutrophils. Both larval and adult neutrophils lacked peroxidase.
