フレイルは、加齢に伴って生理的予備能が低下し、脆弱性が亢進した状態であり、生活機能障害、要介護状態や 死亡などに陥りやすい状態とされる。超高齢化社会の本邦において、フレイルを予防することは、健康長寿を目指 し、高齢者の健康を維持・向上するための重要な課題である。 ドミノ倒しのような負の連鎖によってフレイルが増悪することが提唱されている。すなわち、社会とのつながり が低下して社会的な側面が脆弱化すると、精神心理的な側面の脆弱化ならびに筋力低下を含む身体的側面の脆弱化 が促進され、要介護状態に至る危険性が高くなる。そのため、このドミノ倒しを防ぐには、最初のピースである社 会とのつながりを維持することが鍵と考えられる。 実際、ボランティア活動で社会参加の機会を増やすと、社会とのつながりが維持され、認知機能が改善されることが報告されている。さらに、運動、栄養と社会参加を含む複合プログラムによっても、社会とのつながり、栄養 状態ならびに身体機能が保持され、フレイルや要介護状態へのドミノ倒しを防止できることが報告されている。 健康長寿を達成するためのフレイル予防の有効な戦略として、フレイルドミノのはじまりである社会とのつながりを維持することのできる社会参加の役割は大きい。
Frailty is a condition that occurs as a result of aging-associated decline in physiological reserve and increased vulnerability, resulting in high risk of disability and death. Prevention of frailty is an important issue in Japan as it faces a super-aging society in order to achieve healthy longevity among elderly people. Frailty is suggested to be exacerbated by the domino effect. Deterioration in social aspects is followed by physical as well as psychological deterioration, leading to an increased risk of disability among elderly people. Thus, maintaining social ties is the key to preventing the domino phenomenon. It has been reported that increasing opportunities for social participation through volunteer programs resulted in maintenance of social ties and improved cognitive function, and that programs that combined exercise and diet with social participation were effective in the maintenance of nutritional status and physical function, in addition to social ties, thus preventing the domino phenomenon. Thus, social participation that can maintain social ties could be an effective strategy to prevent frailty and promote healthy longevity.
