介護老人保健施設の摂食・嚥下機能低下者への食形態に関する取り組みの実態

わが国では超高齢社会を迎え、脳卒中の後遺症などにより摂食・嚥下機能低下による低栄養や誤嚥性肺炎が課題の1つとなっている。介護施設の先行研究から、食形態を表す言葉や摂食・嚥下食の基準が統一されていないことが報告されているため、本研究では食形態に関する取り組みの実態を把握することを目的に、山口県内の介護老人保健施設63 施設の管理栄養士・栄養士に無記名自記式の質問紙調査を実施した(回収率:約55.6%)。調査内容は、食形態の使用基準とその理由、食形態の提案に関与する職種、取組みに対する満足度等4項目である。その結果、嚥下食ピラミッド(2004)の使用は約3割にとどまり、施設独自の基準が約6割を占めたが、食形態への取り組みの満足度は、嚥下食ピラミッド使用施設の方が有意に高かった(p<0.01)。食形態の提案には、看護師が最も多く、次いで管理栄養士、介護員、医師が関与していた。今後食形態について病院、施設、在宅との連携の重要性が増す中、その名称や段階の統一が図られることは、施設内外の食形態に対する共通理解を得るためにも重要であることが示唆された。

As Japan enters the era of a “super-aged” society, a decline in eating and swallowing functions due to the after-effects of stroke has led to challenges of malnutrition and aspiration pneumonia. In the previous research of long-term care facilities in Yamaguchi Prefecture, reference of words for “eating” or “swallowing food” represented on the form were not unified. In this study, for the purpose of understanding the reality of efforts related to diet, a further anonymous, self-administered questionnaire survey of dietitian-nutritionists in 63 health care facilities for the elderly in the Prefecture was carried out
(recovery rate:about 55.6%). The survey content included the reasons for selecting the criteria of meal contents, occupations of the staff involved in the proposal of the meal styles, a four-point satisfaction scale, etc.
